fix: Use # to denote line number in stack traces (#6114)

This makes the URLs in stack traces actual URLs to the files.

Editor assessment

The subject states the change precisely: stack traces use # to denote line numbers. The body earns a top mark by explaining the consequence with concrete before and after URLs: stack-trace entries become actual clickable file URLs. That is motivation and evidence in four lines, exactly what a body should teach. Type fix is right, and the absent scope is tolerable for output formatting that spans the CLI. No footers needed. Hygiene is clean, with the capitalized opener a tolerated choice. A genuinely instructive message.
